Dialysis Journeys

We produced a series of four journey maps depicting the experience of dialysis care, including end of life stages. These journeys are real stories of patients who received dialysis care, and were used by the hospice team to better communicate the journey, challenges, and opportunties to funders and decision makers. The journeys were drawn digitally in real time while patients shared their stories, and updated afterward during a round of revision to ensure they were accurate and clear.
